Find on this page the documents you need to fill in before entering the water with me. If you could return it within 24hrs of your first session as this allows me to prepare your sessions in advance. Once we have all the safety bits out the way we can get to the more exciting bits in the water.

• I certify that I am physically fit and have no pre-existing medical conditions that would affect me swimming in open water. (Please discuss anything noted on the medical form with Alysha before arriving on the day.)

• I acknowledge all risks associated with swimming in the sea.

• I agree to swim at my own risk and understand the dangers associated with swimming in open water, with the potential for serious personal injury, property loss or illness.

• I will make sure that I enter and exit the water safely as advised by the organisers.

Skins Waiver

The Cornish Mumaids strongly recommend the use of swim wetsuits by all Swimmers when participating in open water swimming training. Wetsuits provide: | Additional buoyancy (swim wetsuits are not lifesaving, but they will help you to float in the water) | Protection against cold water temperatures and anything sharp you may come in contact with when entering or exiting the water. |Non-use of wetsuits may be agreed on a case-by-case basis, provided that you are able to satisfy safety personnel and/The Coach that you are safe and competent to swim without a wetsuit, based on an acceptable level of experience of swimming in cold, open water conditions previously. 2. You have read, understood and signed the declaration below. You may be asked to produce evidence of this or complete an open water swim test in a wetsuit. Waiver Declaration I understand the risks associated with swimming in ‘deep’ open water, and by choosing to swim without a wetsuit may mean that some of these risks are increased, particularly with regards to reduced protection from cold water conditions and the potential onset of cold water shock and hypothermia. I hereby waive any and all rights of action against The Cornish Mumaids for any injuries and other actionable events, arising from my decision to swim without a wetsuit. Please give details of any medical conditions that may affect your ability to swim physically or mentally. In cold water and/or your participation in a swim coaching session’s, if you have a condition that may be affected by cold water immersion, open water swimming, and accessing the location you must consult your doctor before a session. If after consulting your condition changes, please notify me and seek further medical help before continuing.
